The Portuguese guided Chelsea to the Premier League title this term, his third in charge of the club after successes in 2005 and 2006.
Mourinho was named the Barclays Manager of the Season just last week and has now won the prize for the first time, voted for by his peers.
Chelsea's Eden Hazard won both the PFA and FWA Player of the Year prizes earlier this month and now Mourinho has made it a clean sweep at Stamford Bridge.
Southampton's Ronald Koeman and Swansea chief Garry Monk were both tipped for the award but were pipped by Mourinho.
Sir Alex Ferguson was on hand to present the prize, although the Blues boss was not in attendance due to Chelsea's end-of-season awards on the same night.
The 52-year-old did send a video message, in which he praised the "boss of bosses" Ferguson.
Bournemouth manager Eddie Howe was named the Championship Manager of the Year after guiding the Cherries to promotion.
Bristol City's Steve Cotterill claimed the League 1 prize, with Wycombe boss Gareth Ainsworth winning the award for League 2.
Hazard meanwhile added to his trophy haul with Chelsea's Player the Year and Players' Player awards.
The Belgian played an influential role in his side's league triumph and was rightly recognised, while Kurt Zouma picked up the Young Player prize.
Chelsea also paid tribute to their late life president Lord Attenborough, who was credited with the Special Recognition award.
